I give permission for Stories by Elena, Adriana Zabala Voice & Music Academy, and Mandel Public Library to photograph or record my child during auditions and the talent show for promotional, educational, and community purposes.

I understand that participation in this event is voluntary and agree to release the organizers, sponsors, and venue from liability related to participation in the event.
Audition Date: Tuesday, September 8, 2026 Audition Time: 5:00 PM – 8:00 PM Location: 1850 Forest Hill Boulevard Lake Clarke Shores, Florida 33406 Participants will receive a scheduled audition time after registration.
Show Date: Sunday, September 20, 2026 Time: 1:00 PM – 4:00 PM Location: Mandel Public Library
